Rhinoplasty

Rhinoplasty, or surgery to reshape the nose, is one of the most common of all plastic surgery procedures. Rhinoplasty can reduce or increase the size of your nose, change the shape of the tip or the bridge, narrow the span of the nostrils, or change the angle between your nose and your upper lip. It may also correct a birth defect or injury, or help relieve some breathing problems. The best candidates for rhinoplasty are people who are looking for improvement, not perfection, in the way they look with realistic expectations.

Before rhinoplasty surgery

Prior to surgery a detail medical history is taken and a careful examination is conducted. The physician and patient discuss procedure, anesthesia type, possible risks and complications as well as the incision site. Pre-operative instructions may include the elimination of certain drugs and smoking habits. While you’re making preparations, be sure to arrange for someone to drive you home after your surgery and to help you out for a few days if needed.

Rhinoplasty usually takes an hour or two, though complicated procedures may take longer.

With general anesthesia your nose and the surrounding area will be numbed; you’ll be asleep during the surgery.

After surgery there is swelling and bruising, your nose may ache, and you may have a dull headache. You can control any discomfort with the pain medication prescribed by your surgeon. A small amount of bleeding is common during the first few days following surgery Plan on staying in bed with your head elevated (except for going to the bathroom) for the first day. Applying cold compresses will reduce this swelling and make you feel a bit better. Some subtle swelling-unnoticeable to anyone but you and your surgeon-will remain for several months.

During surgery the skin of the nose is separated from its supporting framework of bone and cartilage, which is then sculpted to the desired shape. The nature of the sculpting will depend on your problem and your surgeon’s preferred technique. Finally, the skin is redraped over the new framework.

Many surgeons perform “closed” rhinoplasty from within the nose, making their incision inside the nostrils. Others prefer an “open” procedure, especially in more complicated cases; they make a small incision across the columella, the vertical strip of tissue separating the nostrils.

When the surgery is complete, a splint will be applied to help your nose maintain its new shape. Nasal packs or soft plastic splints also may be placed in your nostrils to stabilize the septum, the dividing wall between the air passages.

Rhinoplasty risk and complications

Complications connected with the surgical procedure are rare: however there are inherent risks connected with every surgical procedure. There is always a possibility of complications, including infection, nosebleed, or a reaction to the anesthesia. You can reduce your risks by closely following your surgeon’s instructions both before and after surgery. In about one case out of ten, a second procedure may be required-for example, to correct a minor deformity. Such cases are unpredictable and happen even to patients of the most skilled surgeons. The corrective surgery is usually minor. It is possible to develop tiny red marks and “spots”, this can be the result of blood vessels that may have burst under the skin’s surface during the surgery. Although this is extremely infrequent it can happen and the “spots” may not ever go away.

Healing after rhinoplasty is slow and gradual with subtle swelling sometimes persisting for months. However, patients can usually expect to return to work or school within a week or two and to more strenuous activity within two to three weeks.

With the exception of changes associated with aging, results are permanent.

  • Description: Arnica montana or Leopard’s Bane is a perennial herb, growing close to the ground. The leaves form a flat rosette, from the center of which rises a flower stalk, 1 to 2 feet high, bearing orange-yellow flowers. The rhizome is dark brown, cylindrical, usually curved, and bears brittle wiry rootlets on the under surface.
  • Habitat: Indigenous to Central Europe, in woods and mountain pastures, although it has been found in England and Southern Scotland.

Arnica Montana is an herb which grows wild in the Swiss Alps and has been used as a part of European herbal medicine for over a thousand years for the reduction of bruising and swelling and to shorten the recovery period after physical trauma.

Every Plastic Surgeon should inform each of their patients prior to surgery that bruising and swelling are natural consequences of the surgical procedure which the patient has decided to undergo. Each patient will respond individually with more or less bruising and swelling, but each patient will experience both of these symptoms in some degree. There is more or less bruising and swelling depending on many factors including, the type of procedure, the age and overall health of the patient, whether the patient is a smoker, the patient’s inherent tendency to bruise easily, and many others. Arnica montana has been shown to dramatically improve the tendency to bruise and cuts significant swelling almost 80% if taken in pharmaceutical grade strengths.

Usually rhinoplasty is performed at the earliest, 13 or 14 years of age in girls and 15 to 16 years of age in boys. Reason being, and it is a known fact, girls physically mature faster than boys and the collective goal is to perform surgery when at least 90% of the growth is complete. There are major growth centers in the nose that affect the growth of the face.

In cases of severe disfigurement due to accidents, surgery is performed in an attempt to restore the alignment of those centers. For lesser deformities surgery is deferred until after the teen stops growing. Rhinoplasties performed on teenagers and young adults, it seems, are often beneficial to the social development and self confidence of the individual.

In other words, this depends upon the stage of growth in the face as well as the gender of the patient. A qualified surgeon can better help you upon personal examination

Usually, there is no visible scarring unless it is an open rhinoplasty. Some surgeons perform all of their rhinoplasties, open — some, all closed. It solely depends upon the surgeon. You will find that most surgeons tend to disagree when it comes to technique. They either prefer open or they prefer closed, OR if they are highly skilled, know that it is case-dependent and different patients have different needs. If it is an open rhinoplasty the scar would be on the columella (the skin that separates the nostrils) sometimes resembling a straight line or a flattened z.

You can expect swelling, especially in the tip if you are having tip work performed. The swelling usually begins to subside within the first month but the end result may not be seen until at least 9 months [post-operatively. Although this time period tends to lean towards a year and over.

A nasal columella is the external, and sometimes partial internal, fleshy section of the nose which separates the nostrils. When it is referred to being a hanging columella this section is often prominent or hangs down.

No, there are no alternatives for cosmetic purposes. You can use strips that open the airways if you have problems breathing, but they are only a short term solution.

It’s best to make arrangements for someone to drive you home after surgery.
